We strive to make your visit with us as quick and caring as possible. The first step is to contact us for an appointment, and please visit our "What to Expect" section below for more information. Each step along the way, you can count on fine, personalized outpatient care from our specialized and highly skilled medical, nursing and support staff.

When you contact us for an appointment, we will ask for the following information:

  • Full name including any middle initial
  • Address
  • Home phone number
  • Work phone number
  • Cell phone number
  • Date of birth
  • Primary care and/or referring provider’s name
  • Your primary and secondary insurance information
  • What symptoms you are having

We will offer you the next available consultation appointment. We will automatically request your medical information from your primary care provider prior to your visit. If you have any relevant radiology procedures, please let us know when you schedule so that we may request the reports. If you have had any recent hospitalization for GI issues, please let us know which hospital.

When you check in on the day of your appointment, you will have forms to complete, for your convenience you may download these forms from our website and complete them ahead of time and bring them with you to your appointment . The forms include:

  • A patient medical/surgical history form, including your history, medications, allergies, and family history.
  • A HIPAA form which you will need to sign if you want us to be able to speak with anyone else about your medical records.
  • A registration form for your demographic information.
  • A financial policy form.

We will make a copy of your insurance card(s) for our files. Your copay is due on the date of service. If you have any questions about your financial responsibility, please contact our billing department.

After the visit, we will bill your primary insurance. And as a courtesy, we can bill your secondary insurance carrier if you have provided the information when you registered for your appointment.

If you owe anything after your insurance company(s) have responded, we will send you a bill. All bills are due upon receipt and may be paid by check, cash, VISA, Mastercard or the Discover card. The statement shows which insurance we have billed for you. If there is an error, please mail a copy of the front and back of your insurance card with a request to bill the correct insurance carrier. You may also fax the corrected info and request to our Billing Department at (239) 593-6202.

Our office does accept Medicare Assignment, which requires us to submit the Medicare claim on your behalf, and the Medicare payment is sent directly to our office.
